Let’s Get You Scheduled

Step 1

New Patient Forms

To ensure your first visit is as expedient as possible we need to have you complete our New Patient forms before we can get your visit scheduled. If you are seeking a 2nd opinion you will need to complete our new patient forms as well.

Step 2

Call to Schedule

Once you submit your forms, call our office to book your appointment. We will call you within one week of receiving your forms if we do not hear from you.

Step 3

Comprehensive Exam & Consultation

At your first visit, Dr. Nygaard or Dr. Poovey will complete a thorough oral health evaluation. They will examine all aspects of your oral healthcare needs, review your oral heath objectives and review any available treatment options with you. Plan to spend just over an hour with us at your first visit.

Step 4

Treatment Options

At the end of your visit, you will be provided treatment options based on the doctor’s recommendations. You will have time to ensure your questions and concerns are answered before you decide what is best for you.
